A complete application includes the following:

a. A completed, signed application form.

b. All applications must be accompanied with a non-refundable application fee of $100.00 Canadian ($200.00 Canadian for international students), payable to Calgary Oriental Health Ltd.

c. All applications must be accompanied with a deposit of $100.00 Canadian, which will be credited to the unpaid tuitions.

d. The current resume.

e. A personal letter expressing your reasons for pursuing the training.

f. Two (2) written references.

g. Copies of official transcripts.

h. One (1) recent photograph.

i. Copies of all diplomas, certificates, or degrees.
